It helps to bring a valid photo ID, your child's insurance card, a list of any current medications or known allergies, and any relevant medical history or forms, such as school or sports physical paperwork. If your child has had vaccinations at another provider, bringing those records along can help us make sure they're up to date. For newborns, any discharge paperwork from the hospital is also useful.
